802.11a card with external adapter?

Yes, but that card does not have an internal antenna. The antenna sticks
out beyond the cardbus holder. Otherwise, the SMC I suggested would do
the job just fine, as would many other cards.

Maybe the original poster wanted a wifi card with integrated/embedded
antenna (not internal to the card, but also can't be removed) that also
had a RF connector for an external antenna.

Switching receive antennas isn't a big deal since the power is low. I
don't know how well a card can switch a transmitter path.

Personally, I wouldn't want a card that had an antenna sticking out out
all the time if I am going to connector a RF cable to it. The cable can
put stress on the connection to the card. I rather have the stress go to
the edge of the card rather than a piece of plastic sticking out of
the card. It isn't that hard to remove the antenna, though I suppose it
is possible to lose it.

If you want to use netstumbler with this unusual internal/external
switching scheme, you would need to make sure the driver can select the
antenna independent of netstumber or whatever non-standard interface you
are using.
